Our client is looking for an innovative, passionate and professional individual with excellent interpersonal and organisational skills to deliver our post-stroke review service in Telford and Wrekin.

Position: Stroke Support Coordinator – Post Stroke Reviews

Location: Telford (Office based)
Job type: Fixed term maternity cover until 1 October 2019

Hours: 30 hours per week
Salary: circa £19,800 per annum

This is an exciting opportunity to support our clients work with stroke survivors and their families following stroke.

Reporting to the Support Manager, the Stroke Support Coordinator will complete post-stroke reviews with stroke survivors at six-months and twelve months following their stroke. This will enable you to identify any unmet needs and to work together with the stroke survivor and their family to address these as required.

You will have a proven track record of working collaboratively with a range of organisations, clinicians and professionals as well as experience working within the health and social care sector.

You will have excellent IT skills and a strong understanding of how to use data to improve quality.

The use of a car with business use cover and the ability to drive is essential to be able to fulfil the requirements of the role
