Everything we do is informed and inspired by them.  Position: Dementia Support Worker  Location: Hambleton & Richmondshire  Contract type: Part Time, Permanent  Hours: 21 per week  Salary: £12,207 actual per annum  Closing date: 02 May 2019  Interview date: 13 May 2019  About the role:  The role is to support people affected by dementia in the Hambleton and Richmondshire area and is outlined in the attached job description.  As a Dementia Support Worker you will have the rewarding opportunity to provide support and guidance to people with dementia and their Carers; helping to maintain their independence; improving their sense of well-being; and putting them more in control of their lives. The service is person-centred and based upon personal circumstances and support needs.  You will offer support and guidance to your clients in a variety of ways; at service offices or at other locations; face to face; by phone; letter or email. You will also assist people living with dementia to access other services, providing signposting and referrals, as well as facilitating support groups and other group activities.  You will maintain accurate, up to date client support plans and client contact records and you will seek and record client feedback to understand whether needs are being met, and to plan any necessary adjustments. You will work as part of the information and support team, providing cover for colleagues at agreed times or in unforeseen circumstances, as appropriate.  It is imperative to comply with data protection regulations, ensuring that information on clients remains confidential and to work in a manner that facilitates inclusion, particularly of people with dementia.  About you:  Are you an enthusiastic, inspiring and resilient individual? Are you self-motivated, passionate and an outgoing individual committed to putting people affected by dementia at the heart of everything we do?  We are looking for individuals educated to NVQ level 3 or equivalent (or demonstrable experience) to join our team in Tees Valley Hambleton & Richmondshire. The role reports directly to the Services Manager and will require you to have excellent non-judgmental communication, with the following skills (those marked with a * are desirable):  -Good organisation and time management skills  -Experience of face-to-face service delivery  -An understanding of dementia*  -An understanding of the needs of people with dementia and their carers *  -Experience of working with statutory and voluntary agencies *  -Knowledge of relevant legislation, including the Health & Safety and the Mental Capacity Act *  -This role involves regular use of IT packages, therefore a good standard of IT would be desirable.  -Experience of record keeping and report writing  -An understanding of the need for client confidentiality  Our employees work hard every day to make a true difference in people's lives.
