Transition Support Advisors

Successful candidates will provide support, guidance and a key working role to 16 and 17 year old Looked After Young People in Semi-Independent supported accommodation. The role requires the post holder to contribute to the transition from care to independence, as part of a dedicated team working to improve their life chances of Care Leavers

Successful candidates will report to the Team Manager and be responsible for contributing towards:

* A safe environment in which young people are safeguarded from harm, abuse, neglect and exploitation;

* To support young people towards achieving required independent living skills;

* Ensuring young people have their needs met and their welfare promoted;

* Maintaining the highest standards of professionalism;

* Providing a Key Work role to Young People

* Ensuring young people maintain their own personal space and contribute to cleaning communal spaces;

* Maintaining records in accordance with Westhill policies and standards;

* Effective multi-agency practice;

* Maintaining positive community relationships, with creativity and diplomacy.

Requirements:

· 2 year’s experience with working with young people within a residential/supported accommodation/youth work or social work setting.

· NVQ 3 in Health & Social care relating to Young People (NOT DOM CARE).

· Relevant qualification to the field of working with young people.

· Ability to Lone Work.

· Fluent English speaking.

There are full time posts of (40 hours a week) available (part time posts considered). Posts are subject to day and night time shift patterns
