We’re looking for an innovative, passionate and professional individual with excellent interpersonal and organisational skills to join our clients busy stroke support team in Sandwell, as a Support Coordinator.

Position: Stroke Support Coordinator

Location: Sandwell (office based)

Hours: 35 hours per week

Salary: circa £23,100 per annum

Contract: Permanent (our services are contracted; we currently have funding for this contract until 31 March 2020)

The role will be office based in the West Bromwich office with regular travel across the Borough.

Reporting to the Stroke Support Manager, the Stroke Support Coordinator will:

 

  • Support new stroke survivors and their carers from hospital discharge into the community.
  • Provide personalised information, advice and support.
  • Support clients to make informed lifestyle changes which will help them to prevent further strokes
  • Carry out formal six month reviews post stroke
  • Be accountable for volunteers, so experience of recruiting and managing volunteers would be an advantage.

 

About you

The Stroke Support Coordinator will have experience/background in:

 

  • A caring profession with experience of supporting people with disabilities.
  • Working collaboratively with other organisations to achieve joint ambitions.

 

You will have excellent IT skills and demonstrate a flexible approach to your role.

The use of a car with business use cover and the ability to drive is essential to be able to fulfil the requirements of the role.
