Social Worker - Adults CHC - Part Time (3 days) - Wiltshire - £28-30 per hour

Our client in Wiltshire are looking for a specialist CHC social worker to be part of MDT meetings and help to make decisions on the packages of care.

In this role as a Social Worker you will work on the continuing health care team for social care, therefore knowledge of the continuing healthcare process and experience of completing decision support tools in a multi-disciplinary meeting is preferable.

This role will involve the following:

Picking up referrals when CHC funding has ended, as a result of application of the Decision Support Tool
Putting support plans in place
Working in a fast paced environment as this is time-limited work
Working primarily with older people, as well as some younger adults too
Carry out DSTs as part of a multi-disciplinary meeting alongside Individual commissioning nurses from CCG CHC team.
As a CHC Social Worker you must be:

HCPC registered
Eligible to work in the UK
A full UK license holder
Qualified as a Social Worker
