About The Role
Working closely with the Home Manager and within the framework provided by the Head of Wellbeing your role will involve:-
- Delivering the HC-One Wellbeing programme across the home, ensuring a balanced range of activities that stimulate the mind, body and soul, to include clubs and community engagement.
- Involving Residents in the planning and delivery of the HC One Wellbeing Programme in the Care Home.
- Empowering Residents to co-produce their Wellbeing care plans and identifying outcomes to meet their social, psychological and emotional needs.
- Ensuring observable and measurable Wellbeing activity is recorded daily for each Resident and regularly reviewed.
- Reporting any changes in Residents' physical or emotional condition to the Home Manager or Person in Charge of the Residents' community.
- Planning and delivering Wellbeing initiatives based around the life story and wishes of the Residents to enable a personalised and meaningful quality of life.
- Providing 1-1 Wellbeing activity relevant to a Resident's individual needs regardless of the severity of a Residents ill-health, dementia or disability.
- Facilitating a range of self-directed activities that Residents can engage in outside of planned activities.
- Producing a balanced weekly programme of Wellbeing activity covering 7 days that meets the individual and group needs of the Resident community.
- Encouraging Colleagues, Relatives and Friends to participate in the Care Home's Wellbeing programme and promoting a relationship centred ethos.
- Developing and/or assisting with fundraising activities and budgeting for the provision of entertainment, materials and outings.
- Inspiring and supporting Colleagues to understand the aims and objectives of the Wellbeing framework.
- Assisting the Home Manager during interviews for Volunteers who may assist in Residents' Wellbeing.
- Encouraging and overseeing Volunteers to carry out their work to enhance the provision of Wellbeing for Residents.
- Engaging with the local community to promote the Residents' Wellbeing and the role of the Care Home as a local hub.
About You
You will have
- Knowledge of developing activity programmes
- Knowledge and understanding of promoting Residents' Wellbeing
- Basic IT skills
- Good written and verbal communication
- Accurate record keeping skills
- Previous experience working with vulnerable adults
This is a part time role and will include weekend work
