Position: Dementia Navigator
Location: Eastleigh, Hampshire
Contract type: Part Time, Fixed Term Contract
Hours: 25 per week
Salary: £14,260 to £14,835.71 depending on skills and experience

About the role:

We are looking for an enthusiastic individual to join our Team who can provide a highly responsive signposting and support service to people with dementia living in Southampton. You will also be expected to provide an extended information and signposting service to immediate carers who are referred to the service.

Other responsibilities will include marketing the Dementia Navigator Service in partnership with SO:linked and developing a network service of support alongside health care professionals. This will enable you to provide structured support as required throughout the person’s life with dementia.
