Support Coordinator

This is an exciting opportunity to join the team in Birmingham and Solihull. We’re looking for an enthusiastic, motivated and well organised individual to join the organisation in delivering a Stroke Recovery Service across the region, as a Support Coordinator.

Position: Stroke Support Coordinator

Location: Birmingham and Solihull (Office or home based considered)

Hours: 25 hours per week

Salary: Circa £16,500 per annum

Contract: Services are contracted; there is currently funding for this contract until 31 March 2021

The Role

Reporting to the Stroke Support Manager, the coordinator will work with the local Health and Social Care services to receive referrals and support stroke survivors and their carers. The coordinator will provide personalised information, advice and support with regular reviews throughout, this can also include a formal six-month post stroke review.

Key responsibilities will include:

  • Completing Needs Assessments with stroke survivors and supporting them to develop a stroke recovery plan.
  • Undertaking regular reviews throughout and on some occasions a formal post stroke six month review.

About You

As Support Coordinator you will:

  • Have excellent IT skills and demonstrate a flexible approach to your role.
  • Have a background in a caring and/or charity profession supporting people with disabilities. You will be able to demonstrate previous experience of working collaboratively with other organisations to achieve joint ambitions.
  • Experience of recruiting and managing volunteers and community engagement is a benefit.

It is optional for this role to be home based, please see the homeworking agreement for full details, extensive travel is required across the area and occasionally further afield.

This role is either office or home based, but extensive travel is required across Birmingham and Solihull and occasionally further afield.

The use of a car with business use cover and the ability to drive is essential to be able to fulfil the requirements of the role.
