Position: Social Worker - Hospital Discharge
Location: Rutland - Oakham
Salary: £25,799 to £37,788 inclusive of market supplement
Hours: Full time, 37 hours over 7 days (Flexible/Agile/home working)
Duration: 18 months fixed term contract
Benefits: Minimum of 24 days annual leave (pro-rata), with a further 5 days added after 5 years’ continuous service, plus 8 days paid bank holidays, a defined benefits pension scheme, payment of fees for membership to one professional body where this is a requirement of your role, enhanced maternity, paternity and adoption pay, special and family leave provisions, occupational health service, lunchtime yoga, circuits and cycle rides, as well as discounted swimming sessions and flexible working policies to enable you to balance work and home commitments

The Role
As a Social Worker you will complete social care assessments in respect of residents, when either in hospital or within their own homes, and to facilitate the use of personal budgets and/or arrange/commission subsequent packages of care, supported by the wider team.
The role offers manageable caseloads, learning and development, and regular reflective supervision to focus on you. You will be encouraged to continually improve practice to ensure service users’ interests are always protected and promoted. To support you further, your Social Work England registration and DBS costs are covered by the County Council.
