Support Coordinator

We’re looking for an innovative, passionate and professional individual with excellent communication and organisational skills to join the Stroke team in Surrey.

Position: Stroke Support Coordinator

Location: Surrey - Home based, with regular travel across the designated work area of Surrey

Hours: 21 hours per week

Salary: Circa £13,860 per annum

Contract: Services are contracted and there is currently funding for this contract until 1 July 2021 with the aim being contract renewal.

The Role

Reporting to the Stroke Support Manager, the Stroke Support Coordinator will:

 

  • Support stroke survivors and their carers from hospital into the community.
  • Provide personalised information, advice and support.
  • Provide regular reviews to support people in establishing and achieving their own personal goals.
  • Facilitate six-month review meetings with stroke survivors and to produce accurate six review reports
  • Recruit, train and manage a team of volunteers that will support the service.

 

About You

Applicants are likely to have:

 

  • A background in a health or caring profession, ideally with experience of supporting people with disabilities and stroke.
  • Excellent IT skills and a strong understanding of how to use data to improve quality.

 

A flexible approach to your role is essential, as is willingness to travel. The use of a car with business use cover and the ability to drive is essential to be able to fulfil the requirements of the role.

Appreciating much of this role will be spent travelling in Surrey; the role offers Make Your Week Work for You and Those We Serve benefit, which in agreement with your line manager offers the opportunity to work flexibly from home. Daily travel time and expenses are claimed over your usual commute to your designated work area of East Surrey, this area is periodically reviewed. Home working and travel will be discussed further at interview along with working hours.
