Do you love helping people in need? We are looking for a passionate, experienced and enthusiastic person with great people skills to join our Independent Living team in Pembrokeshire.

You will provide a flexible need-led care package that will support individuals aged 18 and above for a 'time limited' period within their own homes in order to either avoid inappropriate 'social' admission into hospital or to facilitate earlier discharge from hospital. This can include all aspects of low-level Personal Care, light housework, meal preparation and prompting of medication.

The role will involve you providing practical and emotional support to service users in their own homes. Due to the nature of this service, this role can be emotionally and physically demanding. But it can also be rewarding; you are helping vulnerable people across the community.

The service operates daily, between 8.00am and 9.00pm. There are a variety of shifts and hours available to our staff.

The Independent Living (IL) & Crisis Response (CR) service (IL/CR) supports people in the space between hospital and home and responds to the needs of people in crisis following an emergency; maintaining an extensive network of external relationships across health, social care and emergency services.

This role could be for you if???
  • You can make things great. You know how to improve service quality for the benefit of users.
  • You're professional. You can deal with queries in a diplomatic and confidential manner.
  • You are locally in Pembrokeshire and can easily travel within the area.
  • You love being flexible. You sometimes may be required to support at short notice, at weekends etc.
  • You have experience of working with or providing personal care for dependent adults either at home or in a working environment.
  • You have a full driving licence with access to own vehicle (please note that, due to vehicle insurance policy requirements, applicants must be of legal age, and have 2 years' experience)

This role includes working face to face with service users and social distancing will not be able to be maintained at all times therefore, it may not be suitable for people that may be at higher risk of harm from Covid-19.
