Job overview
An exciting opportunity has arisen within the Rapid Response Team for full time x 1 vacancy with the Band 5 Community Staff Nurse role.
The desired role is for proactive candidates who are eager to commit to the 2 hr response criteria to patients in the community with the aim of preventing hospital admission operating 7 days per week 8am to 8pm who might otherwise face unnecessarily inappropriate admission to hospital
This role will exist within a team of Advance Clinicians where there is great opportunity for those nurses wanting to progress within the Advance Nurse care pathway.
Community, admission avoidance and DN experience would be ideal but all candidates to be considered
Main duties of the job
To undertake a focussed person-centred approach, adhering to
2 hr crisis response criteria to ensure quality standard and
delivery of care for patients. To develop safe clinical skills in community setting to proactively manage very high intensity users of health and social care services, with multiple co-morbidities and pharmacy, social, medical and nursing needs and who are at risk of hospital admission.

The successful applicant will have contact with patients and service users. It is expected that in early 2022, being vaccinated against Covid 19 will become a legal condition of employment unless an exemption applies. We will ask therefore for proof of vaccination during the recruitment process.
Detailed job description and main responsibilities
To undertake nursing procedure as indicated for example,
catheterisation, phlebotomy, IV therapy and cannulation in the
patient’s home.
To proactively monitor and manage a patient caseload with
sound clinical autonomous decisions about patient care and
seeking support from colleagues as appropriate.
To confer, as appropriate and work with the patients GP, Clinical
Lead, ANP, NP, ACP and/or Out of Hours GPs to ensure there
is a robust communication around clinical issues as indicated.
To sensitively deal with distressing or emotional circumstances
regarding patient care, and communicate information,
sensitively, requiring empathy and reassurance, to patients,
carers.
To actively seeking GP to review of polypharmacy and other
medication issues in accordance with Department or Health,
Professional and organisational guidelines.
To work effectively as part of a multidisciplinary team working to
support the common goal of safe, efficient care delivery closer to
the patients’ homes.
To empower patients and their families through appropriate
support and education.
To utilise formal and informal reporting mechanisms, including
electronic and handwritten reporting to ensue effective internal
and external communications and to ensure all documentation is
accurate and professional.
Fully support the use of EMIS (iPad) mobile and provide
contemporaneous clinical records.
To continuously challenge practice ensuring a sound evidence
base thus meeting clinical governance requirements.
Promote multidisciplinary team working to provide rapid crisis
intervention, the common goal of safe efficient care to people in
their own home.
Support the Service Lead and Operational Leads in delivering
the Rapid Response KPIs and ensure that capacity is
maintained and timelines managed at times of escalation.
To contribute to partnership working with GP’s, Nursing home
staff and other health care professionals within Bromley
Healthcare, Social Services and voluntary agencies to promote
an anticipatory care planning approach thereby supporting some
aspects of the unscheduled care agenda
To develop good working relationships with colleagues,
specialist HCP and also mainstream nursing services in BHC
and contributing to development of policies and procedure that
cover related services.

- Up-to-date knowledge of current NHS Agenda around community rehabilitation.
- Cannulation skills
- Qualified & competent to carry out male/female catheterisation
- Sound knowledge and understanding of Clinical Governance
Desirable criteria
- Ability to administer intravenous drugs.
- Specialist assessments such as Continence assessments, Digital Rectal examination
Experience
Essential criteria
- Community Work experience
- Experience of working in/with a multi-disciplinary team.
- Actively involved in clinical supervision
Desirable criteria
- Recent experience of administering intravenous drugs
Personal Qualities
Essential criteria
- Car driver with clean current licence and access to a vehicle to use for work
- Keen interest in developing nursing services within Intermediate Care.
- Time management skills and ability to prioritise
