Job overview

This exciting opportunity will see the successful candidate to join a multi-disciplinary team providing short term therapy assessments and reviews within a high quality service to Bromley patients.

 

Main duties of the job

The post holder will hold a small caseload and be responsible for progressing simple rehab programmes, providing basic equipment provision, Walking aid provision, mobility assessments, referring on to external support services and reviewing and discharging patients with overall supervision from a registered member of staff.  

It is accepted that at times the Rehab Assistant will work without supervision, carrying out established plans of care with goals, joint working with senior Physio and Occupational Therapists, facilitating safely and timely discharge from hospital, along with the prevention of hospital, or inpatient/ residential admission.

This role will be predominantly based in the Rapid Access Therapy Team with a focus on patients who have fallen but will also be expected to cover work within all the Urgent Community Response Teams.

Please be aware this post will require 7 day working with flexible work patterns. The post holder must have a valid driver’s license and access to their own car for work purposes.

The successful applicant will have contact with patients and service users. Detailed job description and main responsibilities

The main duties of the role include: 

  • To be responsible for own clinical caseload of patients as allocated by a qualified therapist.
  • To complete new patient assessments, as deemed appropriate by the triage team, integrating occupational therapy and physiotherapy assessments into one holistic assessment.
  • Joint working with Senior Therapists in relation to falling patients.
  • Carry out falls risk assessments in line with completed competencies.
  • To carry out manual handling with patients according to Bromley Healthcare guidelines alongside qualified staff and to be responsible for the safe and competent use of manual handling equipment.
  • To implement and progress patient management and treatment plans within scope of practice and in line with competencies whilst under the supervision of a senior therapist.
  • Assessment for and provision of basic equipment needs in line with competencies. Responsible for referring to an occupational therapist for complex equipment assessments. 
  • To assist in vital sign interpretation - establishing changes from normal parameters and/or baseline measurements i.e. oxygen saturations, blood pressure, pulse, temperature, respirations and take appropriate action.
  • To support patients on a one to one basis (in their own homes) using appropriate rehabilitation techniques which will include, but not be limited to:
    • Mobility work
    • Provision of strengthening programs, indoor and outdoor functional tasks.
    • Practice of activities of daily living
  • Cognitive screening
  • Supporting Home Visits for supported discharge patients
  • Joint visits with Qualified Occupational Therapists/Physiotherapists.

Qualifications

Essential criteria

  • Literate and Numerate in English
  • NVQ3 or equivalent in health or social care subject.
  • Holds a current UK driving license and has access to a car for work purposes.

Desirable criteria

  • ExerciseCertificate from internal/external training e.g. management of long term conditions, falls rehab etc. qualifications, e.g. later life

Experience

Essential criteria

  • Recent experience within a health or social care setting as a rehab assistant/technician or nursing assistant.
  • Experience of working with people who have a variety of physical conditions.
  • Working within a multi-disciplinary team

Desirable criteria

  • Experience of working in the community / lone working.
  • Experience of fitting a range of equipment
  • Experience of completing cognitive assessments e.g. MOCA, AMTS
  • Knowledge of therapy interventions acquired through training or experience within a healthcare setting to foundation degree level

  • Ability to use electronic patient record systems, i.e. EMIS, with relevant training.
  • Ability to use mobile devices for input of electronic patient records.

  • Knowledge and/or previous experience of using Microsoft programmes
