SACMHA - Community Support/Outreach Responsible to Project Leader Part time - 18.6 hours per week £8700 per annum Cover for Support Worker support  The post holder must at all times carry out his/her responsibilities in accordance with the association’s policies and procedures. 1. PURPOSE OF THE JOB To support and assist Service Users in a manner designed to increase their level of independence and self-sufficiency and develop their life skills. This may include advice and information on matters such as benefits, health and general welfare, leisure and community facilities, practical assistance, budgeting maintaining a tenancy. Facilitate and run groups and workshops when appropriate. To maintain a good working knowledge of new work procedures and to take advantage of training opportunities made available. To maintain a good working knowledge of health and safety procedures & fire precautions and operate the correct procedures in the case of an emergency. To provide a range of community support activities, specifically to the African Caribbean population in Sheffield and surrounding areas, as determined by SACMHA. To ensure the delivery of quality operations and/or service, in accordance with SACHMA’s quality assurance systems and quality standards in conjunction with the project leader. To maintain a safe and healthy working environment in accordance with SACHMA’s Health and Safety policies and procedures in consultation with other staff, Service Users and management. To operate within, and promote, SACMHA’s Equal Opportunities policy framework. 2. KEY DUTIES AND RESPONSIBILITIES To liaise with appropriate statutory and voluntary organisations who have an interest in this area of work and who may wish to refer clients. To ensure that information retained in respect of all contact with Users, their careers and other relevant bodies are secure, accurate and up to date. To participate fully in the operation and maintenance of all services currently pro- vided by SACMHA. In conjunction with the project lead to assess and decide on referrals into the service, develop support plans and carrying out Service User risk assessments. To support service users in their own homes and in the community having in mind relevant risk assessments. To support carers directly where appropriate and referral to other organisations as appropriate To offer advice, information, and support to Service Users, enabling them to maintain an existing tenancy or be rehoused into appropriate accommodation. Do we need this? To provide information to service users and carers about other support services available in the city. To provide support with employment, education, housing and social integration within the Service Users’ local communities. To provide advocacy services where appropriate in order to support Service Users in pursuing any grievances in relation to their support, or to facilitate maximum uptake of available welfare benefits. To liaise with participating Housing Associations and other housing providers over the provision of properties. To attend team meetings as appropriate. To liaise and where necessary represent SACMHA at meetings with other relevant organisations and resource points, particularly those concerned with minority ethnic groups and mental health issues. To provide information for internal and external reviews of the work carried out by SACMHA. To attend regular supervision sessions. To undertake any other duties appropriate to the grade and purpose of the job as may be determined after discussion between the post holder, Service Manager and the Board. Candidate Criteria: Ability to communicate well with a wide range of people, verbally and in writing. Ability to demonstrate sensitivity, responsiveness and be confident when dealing with Service Users. Ability to demonstrate a willing and con-structive approach to working with Service Users, carers, relatives and other professionals. A desire to work towards continuous improvement, including undertaking training or other requirements (e.g. NVQ.) Willingness to work in accordance with the Association’s Equal Opportunities Policy. Ability to demonstrate a clear commitment to anti-oppressive practice. Understanding or knowledge of the issues faced by African Caribbean people with mental health problems. Experience or knowledge of the key working process, needs assessment and of individual support programmes. Experience of dealing with, or ability to deal with, difficult or urgent situations involving vulnerable people. Experience of accessing, or ability to access, appropriate services for vulnerable people at short notice. Ability to use IT programmes such as Microsoft Word, or commitment to undertake training in this area. Be of African or Caribbean origin (Part II Section 5.2(d) Race Relations Act applies).
