Our client is looking an experienced Practice Nurses to work as part of a team based in the Birmingham area.
Key Responsibilities:
* To provide high quality, safe and evidence based nursing care and advice to registered patients
* Contribute to and participate in the delivery of a range of chronic diseases with an awareness of the Quality and Outcomes framework.
* To Independently prescribe or provide medicines via patient group directions or in accordance with a prescription by an independent prescriber
* Work in a collaborative manner with other agencies to ensure seamless service delivery for patients Provide health promotion advice on an individual basis
* Provide necessary information relating to a patient’s health or illness and be aware of resources outside the service that may be used to supplement this information
* Demonstrate awareness of cultural or language barriers to health and take action to mitigate these.
* Ensure own clinical competence and updating through ongoing professional development and the active participation in clinical and staff meetings.
* Ensure the treatment room is well stocked, clean and infection control practices and principles meet current national and local infection prevention and control standards.
* Ensure all equipment is fit for purpose and properly maintained.
* To ensure appropriate access to nursing and medical primary care services.
* Regularly review progress on all nursing related and public health performance indicators and proactively manage progress with other key staff.
* To make best use of clinical knowledge and skills existing in the nursing team, identify areas for development and work with the lead nurse to address these.
* To personally deliver or ensure delivery by an adequately qualified team member all primary care services to registered patients including wound care, ear care, immunisation and vaccination, cytology in line with local and national guidance and protocols.
* To deliver personally or support delivery by other appropriately qualified team members the ongoing management of long term conditions ensuring and promoting high standards of care.
* To monitor the work of the health care assistants, delegate suitable work, supervise, assess and evidence the quality delivered.
* To actively support the training and development of clinicians within the centre.
* To actively participate in clinical audit and other governance processes.
* Provide professional and clinical support and advice to other members of the Primary Health Care Team.
* To adhere to the Code of Professional Conduct of the regulatory body i.e. NMC
* Keep professionally updated and be responsible for addressing any unmet personal training needs.
