Key requirements of a Care Worker:
* Must live within a commutable distance of Kings Lynn
* Friendly, reliable people who are driven to provide the best quality of support to adults with Hospital Discharge and Dementia Care
* Experience in the social care sector, with a minimum of NVQ/QCF Level 2 or other professional qualification in Care
* Driving licence, access to a vehicle and a good heart, we can train the rest
Key responsibilities of a Care Worker:
* To be part of a team of professionals based in the community, to support with Hospital Discharge & Admission Avoidance, including providing family carer support, & support for people who are EOL & people with Dementia.
* To treat all carers and people with care needs with respect and sensitivity, recognising the need for confidentiality at all times
* To undertake the tasks specified in the care plan, adapting your practice to the preferences of the individual; this may include personal and social care tasks
