Salary: Band 5 £26,404 - £34,173 per annum

Hours: Full Time

The role:

Are you a Registered Nurse with experience of working within the Community? Do you have good clinical skills and competency in wound care? Are you caring and compassionate, motivated and committed to providing assistance and care within the Community to a range of clients? Do you have the confidence and capability of providing care to our clients without direct supervision?

As part of the multi-disciplinary team you will provide assistance in assessing, planning, implementing and evaluating programmes of care for our clients. You will also be required to assist with the management of caseloads in order to provide a high quality service as well as providing assistance in the supervision, deployment and teaching of staff and student nurses acting as their mentor, preceptor and/or supervisor.

Duties & Responsibilities:

To assist in assessing, planning, implementing and evaluating programmes of care for patients / clients in a defined caseload.
To assist with the management of the caseload.
To provide high quality patient care.
To assist in the supervision, deployment and teaching of staff and student nurses, acting as a mentor, preceptor / supervisor.
To work in the clients / patients' homes without direct supervision.
To work as part of a multi-disciplinary team within an integrated care model.
Application requirements:

Registered Nurse.
Teaching / mentoring qualification.
Community experience.
Driving licence.
Good clinical skills and competency in wound care.
Good communication and written skills.
Good interpersonal skills with colleagues and clients.
Caring and compassionate.
Motivated and committed.

All bonus payments will be paid on a pro-rata basis should the position be offered on a part time basis or if the Appointment begins or ends part way through a holiday year
