Jewish Care - Senior Social Worker - North London - £24,797 - £26,175

The Social Work and Community Support Service (SWCW) comprises 30+ full and part-time workers, based in four locality teams in London and the South East, who work with adults within the Jewish Community. They provide personalised and integrated support offering guidance, advice and advocacy to meet identified health and social care needs of individuals, families and carers, as well as facilitating support groups and other community-based interventions. The aim of the service is to support independence, promote choice and control for people facing difficulties due to ageing, caring responsibilities, illness, isolation, disability, mental health issues or changes in circumstances.

The post-holder will be a member of the North West and Central London Team which is based in Golders Green.

This post is Full Time 36.25 hours per week working, (M-F, 9am - 5.30pm, summer months and 9am - 2pm, Friday's, during the winter months).

Responsibilities as an Senior Social Worker with Jewish Care:

- Take a Senior role in managing the Duty provision and develop an effective service
Supervise social workers and develop the role of the Duty volunteers
- Work in partnership with individuals, carers and relevant voluntary and statutory resources to enable people to use the service, to reduce risk to themselves or others and/or improve their quality of life in a meaningful way
- Be active for initiating and maintaining collaboration with internal and external partnerships, to develop community and group interventions that enhance social and community life for individuals and their families/carers
- Taking a multi-disciplinary approach to provide effective intervention, whilst being pro-active in maintaining effective collaboration.
- Develop knowledge and understanding of how Jewish Care staff support initiatives to deliver a wide range of services in our Community Hubs, Care Homes, Community Centres and outreach programmes
- Take an active role in attending relevant locality and specialist forums
Triage, support and guide people, face to face or over the phone to assess urgency and need
Communicate with clients regarding the next step, to ensure effective case managements when awaiting allocation
- Respond to enquiries in a timely manner, working towards the values of the organisation.
- Record all conversations and assessments on Dynamics CRM
- Participate in a specialist forums developing knowledge and skills with regard to Mental Health, Family Carers, Disability, End of Life Care and Dementia
- Provide information and advice in accordance with current government health and social care guidance, and to conduct holistic assessments for Jewish people living in the community
- Demonstrate confident and effective judgement in making timely decisions to achieve positive change
- Work with and supporting colleagues to identify local services and resources that promote independence, choice and dignity of individuals, carers and families, to enable people to remain within the community as far as possible
- Take an active role in the promotion of collaborative practice in a multi-agency context to ensure people in the community are supported safely, in the best way possible
- Ensure that information gained through community involvement is fed back to Jewish Care, and acted upon, in order to continually improve service provision according to changing circumstances
- As a senior member of the team, being pro-active in supporting the effective management of the team
- Present the work of the Social Work and Community Support Service in different forums.
