Location: Bristol, BS2 8HW (requires travel within the area)
Contract type: Fixed Term until 31st of March 2020
Hours per week: 28
Salary: GBP 15,682 pro rata (GBP 8.62 per hour)

This winter, we need you to help us support to those in need!

At the British Red Cross, it's our mission to help those in crisis, whoever and wherever they are.

Every winter, hospital admissions increase, placing additional stress on health services. Our goal is to ensure people are not unnecessarily admitted to hospital or delayed in being discharged, causing a detriment of their wellbeing and independence.

You will provide reactive, focused and flexible support to adults either following a hospital stay to enable early discharge when clinical needs have been met or to those living in the community to prevent hospital admission.

You will respond to referrals from Clinicians and Community Health Professionals and will be required to have the ability to assess service users' needs, complete and follow a support plan for the individual. In addition, you will be providing practical and emotional support in the service user's own home and signposting to other services where appropriate to enable the individual to live independently in their own home.

This position doesn't involve personal care or administering medication.

The Independent Living (IL) & Crisis Response (CR) service (IL/CR) are dedicated to making lives of vulnerable people a little bit better. We're a small team dotted all over the country that shares the same goal - to help those that need it most.

Also, you should be the following:
  • Educated to GCSE level (or equivalent by experience)
  • IT literate
  • Full driving licence enabling to drive manual transmission vehicle
  • Have a good knowledge of services provided by the NHS and Social Care
