Support Coordinator

This is an exciting opportunity to work with stroke survivors and their families to support them following stroke. We’re looking for an innovative, passionate and professional individual with excellent communication and organisational skills to join a Stroke Recovery Service based in Leicester.

Position: Stroke Support Coordinator

Location: Home Based in Leicester with regular travel across the service area

Hours: 19 hours per week

Salary: Circa £12,540 per annum

Contract: Permanent. Services are contracted; the is currently funding for this contract until 31 March 2022

The Role

The service aims to identify and address the needs of stroke survivors and carers across the stroke pathway, by providing a range of innovative support solutions, supporting them to meet their desired outcomes.

A key part of the role will be to recruit, train and manage a team of volunteers, to support service users and the delivery of the service.

Reporting to the Stroke Support Manager, the Stroke Support Coordinator will:

 

  • Provide personalised information, advice and support.
  • Complete needs assessments with stroke survivors and support them to develop a stroke recovery plan.
  • Provide regular reviews to support people in establishing and achieving their own personal goals.
  • Support clients to make informed lifestyle changes which will help them to prevent further strokes.

 

About You

The Stroke Support Coordinator will have experience/background in:

 

  • A caring profession ideally with experience of supporting people with disabilities.
  • Excellent IT skills to enable you to capture your work on a database and provide support through virtual means as required.

 

You will be organised and able to work independently, to manage your priorities to deliver a quality service.

This role is home based, but extensive travel is required across the local area and occasionally further afield.

The use of a car with business use cover and the ability to drive is essential to be able to fulfil the requirements of the role.
