Support Coordinator

We are looking for an enthusiastic and motivated individual to join the Stroke recovery team in the Swansea Bay Health Board area.

Position: Support Coordinator

Location: Homebased Wales, Swansea Bay University Health Board area

Hours: 28 hours per week

Salary: Circa £18,480 per annum

Contract: Our services are contracted; we currently have funding for this contract until 31 March 2022

The Role

Reporting to the Stroke Support manager, the Stroke Support Coordinator will support stroke survivors and their families and carers by assessing the needs of each stroke survivor and their family.

Key responsibilities with include:

 

  • Supporting people to make lifestyle changes based on their identified risk factors
  • Giving information and helping Stroke survivors to set personal goals and plans to work towards them.
  • Providing support to help build confidence of people enabling service users to regain their control and improve their quality of life after stroke.

 

About You

You will:

 

  • Be required to be IT efficient and have experience of recording data to generate reports
  • Have a background in a caring and/or charity profession supporting people with disabilities, preferably with knowledge of stroke.
  • Be able to demonstrate previous experience of working collaboratively with other organisations to achieve joint ambitions.
  • Have the ability to use basic Microsoft system.
  • Have all-round flexible skills in listening, communication, organisation and resourcefulness.

 

This role is home based, but extensive travel is required across the local area and occasionally further afield.

The use of a car with business use cover and the ability to drive is essential to be able to fulfil the requirements of the role
