Support Coordinator

This is an exciting opportunity to work with stroke survivors and their families to support them following stroke. We’re looking for an enthusiastic, motivated and professional individual with excellent communication and organisational skills to deliver the Stroke Recovery Service across Wiltshire.

Position: Stroke Support Coordinator

Location: Homebased Wiltshire, with extensive travel across the service area

Hours: Part-time, 29 hours per week

Salary: Circa £19,140 per annum (full-time equivalent £23,100)

Contract: Permanent. Services are contracted and there is currently funding for this contract until 31 March 2023

The Role

The service aims to identify and address the needs of stroke survivors and carers across the stroke pathway, by providing a range of innovative support solutions, supporting them to meet their desired outcomes.

• Reporting to the Service Delivery Coach, the Stroke Support Coordinator will:
• Support new stroke survivors and their carers after discharge from hospital into the community.
• Provide personalised information, advice and support.
• Support clients to make informed lifestyle changes which will help them to prevent further strokes.
• Provide reviews to support people in establishing and achieving their own personal goals.

About You

The Stroke Support Coordinator will have experience/background in:

• Have a background in a caring and/or charity profession supporting people with disabilities
• Have experience of working with people who may have additional communication support needs

You will have excellent IT skills and demonstrate a flexible approach to your role.

This role is home based, but extensive travel is required across Wiltshire and occasionally further afield.

The use of a car with business use cover and the ability to drive is essential to be able to fulfil the requirements of the role.
