Support Coordinator

This is an exciting opportunity to work with stroke survivors and their families to support them following stroke. We’re looking for an enthusiastic, motivated and professional individuals with excellent communication and organisational skills to join the Communication Support Service across Hampshire.

Position: Stroke Support Coordinator

Location: Homebased, West Hampshire with regular travel across the service area

Hours: Part-time, 18 hours per week

Salary: Circa £11,880 per annum (full time equivalent £23,100 per annum)

Contract: Permanent. Services are contracted and there is currently funding for this contract until 31 March 2023

The Role

Reporting to the Service Delivery Coach, the Stroke Support Coordinator will:

 

  • Identify and support the needs of stroke survivors and carers across the stroke pathway.
  • Provide a range of innovative support solutions to support them to meet their desired outcomes.
  • Providing practical advice and emotional support in relation to communication impairment.
  • Supporting changes in communication and engaging people in language use.
  • Be accountable for volunteers, so experience of recruiting, training and managing volunteers would be an advantage.

 

About You

The Stroke Support Coordinator will have experience/background in:

 

  • A caring professional ideally with experience of supporting people with disabilities
  • Setting up and running communication groups

You will have excellent IT skills and demonstrate a flexible approach to your role.

This role is home based, but extensive travel is required across West Hampshire and occasionally further afield.

The use of a car with business use cover and the ability to drive is essential to be able to fulfil the requirements of the role.
