Health Visitor - Band 6 Health Visitor- Greenwich + salary incentive

There is a salary incentive attached to this role of £1,000 upon commencement in role.

Main duties of the job  

As a health visitor working in Greenwich you will provide quality face to face contacts for families for all key contacts, work in a diverse borough following the Healthy Child Programme. Health Visitors have regular management, safeguarding, clinical and restorative supervision and support throughout your career and a chance to develop and grow with Bromley Healthcare. Our health visitors work in teams which are dynamic, supportive, friendly and well established. We work closely with GP's, Children Centres and Local Authority Commissioners. There is a commitment of strong visible leadership at all levels. Health Visitors have access to mobile working with laptops and smart phones. Professional progression is encouraged with a programme of training throughout the year which is clinically led andcomplimented by Bromley Children's Safeguarding Partnership training .

Job description
Job responsibilities  

To deliver the Healthy Child Programme by:

  • Carrying out ongoing health needs assessments for all children and families within the community, using professional judgement to allocate each family to the correct level of health visiting care, Universal, Additional Support Offer, Intensive support and Intensive Support Offer for Young Parents pathways.
  • Overseeing the delivery of Universal service key touchpoints by skill mix team members ensuring they are within required timescales, according to the service model, to meet KPI targets.
  • Delivering Additional Support Offer, Intensive support and Intensive Support Offer for Young Parents packages of care to families.
  • Using Outcome measures to evaluate the effectiveness of care delivered, planning and implementing revised packages of care, where required, and closing or escalating packages of care as required for each child.
  • Participating and where appropriate, leading on the Early Help process for children, ensuring timely assessment and that coordinated care is delivered to address the needs of the child and family.

Person Specification Qualifications Essential

  • Diploma/Degree

Desirable

  • Qualified Nurse Prescriber

Professional Registration Essential

  • NMC registration as Nurse or midwife and Specialist Community Public Health Nurse

Training Essential

  • Evidence of Continuing Professional Development

Desirable

  • Evidence of further professional learning. Knowledge of current research

Skills Essential

  • Current knowledge and understanding of reports/legislation affecting community nursing including current PHE directives. Teaching Leadership qualities Management skills Knowledge of the public health role of contemporary health visiting Safeguarding Evidence based practice Computer literate

Experience Essential

  • Current community experience Experience of working across organisational boundaries within health/ social care. Team working / autonomous working Evidence of professional updating Experience of working in multi- disciplinary teams. Knowledge of Healthy Child Programme.

Desirable

  • Clinical appraisor/supervisor experience
