Main duties of the job  

 

The successful applicant would be responsible for driving the service forward in order to enhance holistic, patient centred interventions to patients within the borough of Bromley. Leadership at multidisciplinary team meetings and regularly liaising with external parties i.e. PRUH, specialist nurses, are an essential part of the role. At times there will be an element of case co-ordination in which you will be expected to work alongside the neuro-navigator or in-reach to acute hospitals.

The post covers the borough of Bromley, working predominantly in patients' own homes alongside their families and carers. You will need to have a full driver's license and access to your own car for work purposes.

This is an exciting opportunity to develop a new full time post within an existing service. We are looking for a self-motivated team player who enjoys a challenge but most importantly, puts the patient at the forefront of their care planning. Although close working will be developed with other teams you would be working as an autonomous practitioner and would be responsible for the development and provision of the neuro service. Peer support would be provided with other teams across the Trust. Working hours are Monday-Friday 9am-5pm.

MAIN DUTIES AND RESPONSIBILITES

  • Support the head of service with the development and delivery of strategic and innovative service improvement projects focussed on improving quality of care and patient outcomes.
  • Professionally lead and line manage the team to ensure patient flow from acute trusts through to the community is responsive, effective and continuous. To be accountable for achieving service targets and KPIs including same day patient visits.
  • Lead and extend on the current work being carried out by the SEL neuro core offer working group, ensuring timely implementation of changes and taking a proactive approach to facilitating these changes.
  • To act as the lead AHP on the SEL education steering group meetings. To apply learning and share innovations from this group across teams.
  • To use a range of verbal and non-verbal communication tools to communicate effectively with patients discussing often complex and sensitive issues. To regularly assist with less experienced staff working with patients who may have barriers to communication. To ensure all appropriate communication tolls and resources are in place to meet the patients needs.
  • Provide the head of service with assurance of proactive management of clinical and cost improvement plans, ensuring appropriate actions are taken to mitigate or respond to identified risks.
  • Support staff development from a clinical and non-clinical perspective acting as a good role model. Deliver department plans, which drive efficiency and quality initiatives, across the UCR care group in partnership with other service leads, finance and human resources colleagues.
  • To have sound knowledge of discharge pathways, stoke and neurological clinical guidelines and consistently strive to maintain and increase capacity within the service.
  • To act as a lead advisor and support team members by providing highly skilled, in-depth professional knowledge and clinical expertise through a structured in-service program and supervision support schedule.
  • To manage and assist the team in complex cases using advanced clinical reasoning, critical thinking, reflection and analysis to support assessments using a client centred approach. To manage a complex clinical caseload of patient with long term neurological conditions using evidenced based principles and ensuring specialist level of assessment/intervention.
  • To act as a supervisor and mentor for occupational therapy and physiotherapy students on practice placements. To implement a thorough student induction process and training program.
  • To lead on clinical governance for the service promoting evidence based practices and quality assurance in the service area in order to deliver an equitable, high quality approach to client care.
  • Ensure all staff are competent in the skills required to complete their work duties to the highest standard. To establish, maintain and monitor standards and competencies of clinical care within the team evidencing this through competency based documents.
  • To regularly analyse and evaluate service performance through the use of the service dashboard. To prepare and disseminate reports and data derived from these findings, identifying areas of improvement required and set out a targeted action plan for improvement.
  • To manage the service roster and support admin staff in the process
